Vision

The Ware County Schools Vision Plan with Simple Vision allows you to use any provider, and there are no limits on frequency or services. The plan covers an eye exam, refraction, frames, lenses, and contacts.  The plan pays 100% of the first $125 of eligible vision expenses, then 50% of the next $750 of expenses.  The annual maximum benefit is $500.  The plan excludes LASIK vision correction surgery.

Annual Eye Exams


We encourage all employees to have an annual eye exam with a doctor of optometry, even if you have clear vision.  It’s important to be proactive in taking care of your eyesight.  Many vision problems with discreet symptoms go undetected for long periods of time.  When vision problems are not addressed in a timely manner, they can lead to larger health complications down the road.

 

It is recommended to get an eye exam at least once a year, especially over the age of 40 when the risk of vision problems and other health conditions increases. Eye exams can detect signs of serious health issues such as diabetes, heart disease, high blood pressure, and cancer.   


The American Optometric Association also encourages children between the ages of 6 months and 5 years receive an eye exam by a licensed pediatric optometrist to ensure their vision is properly developing in the early stages of life.  After that, annual eye exams are recommended.

Claims Administration


Most vision providers file the claims with Simple Vision so you can receive your benefits at the point of service.  Your provider can submit the claims to Simple electronically via the member portal, via fax, or via mail. If your provider is unable to file your vision claims, you may provide an itemized receipt to Simple for processing.  Mail to Simple, 2810 Premiere Parkway, Suite 400, Duluth GA 30097

Member Portal


As a Simple Vision Plan member, you’ll receive access to a member portal. As a reminder, the vision plan is administered by 90 Degree Benefits.  This means that 90 Degree Benefits manages the plan eligibility and claims.  You can review your annual benefits, claims information, plan documents, and covered dependents on the portal, and a convenient mobile app is also available.  You can access the member portal here.

SHBP Vision Benefit


If you are enrolled in a SHBP Medical Plan, the plan covers 100% of one routine eye exam every 24 months. The plan does not extend to additional vision benefits such as eyeglasses or contact lenses.
