Critical Illness

Ware County Schools offers a voluntary Critical Illness benefit through Lincoln Financial Group that provides a monetary benefit to help with the out-of-pocket medical and non-medical expenses upon diagnosis of a covered illness. Critical Illness insurance helps you and your family maintain financial security during the recovery period of a serious medical event, such as cancer, heart attack, or stroke.  This plan pays a benefit if the initial diagnosis for a covered illness is while the certificate is in force.  (See certificate for complete details.)

 

Premium Information

Important Documents

Coverage amounts for employees, spouses, and children are as follows:

Employee: $10,000, $15,000, $20,000, or $30,000

Spouse: $5,000, $7,500, $10,000, or $15,000, not to exceed 50% of the employee coverage amount

Child(ren) to age 26:  Automatically covered at 50% of the employee amount elected

 

Employees may elect up to the maximum amount of coverage for yourself and your spouse with no health questions.

Benefits are paid in addition to any other insurance that you may have, and benefits are paid directly to you

This plan pays a benefit if the initial diagnosis for a covered illness is while the certificate is in force.  (See certificate on the Resources page for complete details.)

This product will pay multiple times for the same or different covered conditions (see the certificate on the Resources page for additional information)

Benefits may be used however you'd like.  Typical uses include:

  • Out-of-pocket medical and non-medical expenses
  • Home health care needs and home modifications
  • Recovery and rehabilitation
  • Child care or caregiver expenses
  • Travel expenses to and from treatment centers

Tobacco rates are available during enrollment.  Spouse rates are based on spouse age.

Late entrants are not required to answer health questions, but you do have to wait until the next Open Enrollment period to apply for coverage.

There is no pre-existing limitation on the plan.  Date of initial diagnosis has to be after you are insured with Lincoln Financial.  Reoccurrence only applies to benefits Lincoln Financial has already paid.

Wellness Benefit Included


The voluntary Critical Illness plan includes a wellness benefit of $50 per covered member per calendar year. The benefit covers one test per insured per 12 months. 

 

  • Annual exams by a physician, including sports physicals
  • Immunizations and other health screenings
  • Cardiovascular, cholesterol, and diabetes screenings
  • Imaging, including chest X-ray, mammography, and more
